Support Programme

SWAN Independent Living support programme provides guided and supported care for young people to assist them in their transition from foster and residential care environments to living independently.

Each young person is assigned a key worker that provides a tailored support programme for their individual needs and circumstances, to prepare the young person for the demands of adult life.

SWAN Independent Living provides support from fully trained and vetted staff members, to ensure each young person has the right levels of support in an encouraging manner and environment. The support will consist of direct individual and group work as well as 24-hour telephone support. 

The aim of this support is to ensure:

  • Young people take some responsibility for themselves and the home that they live in;
  • Young people build on the skills they already have, and learn new skills;
  • Young people have opportunities to gain confidence, make new friends and gain a better understanding for living independently;
  • Young people’s views are taken into account and are advocated for, and that they are respected as individuals;
  • Records are kept outlining how the Five Outcomes are achieved for each young person.
The Five Outcomes are:
  1. Being Healthy
  2. Staying Safe  
  3. Enjoying and Achieving 
  4. Making a Positive Contribution 
  5. Achieving Economic Well-being 
SWAN Independent Living's support programme also covers practical elements such as shopping; cooking; personal hygiene; budgeting; money management.
